How does it work?

1) Enroll into Coloplast Care by submitting the form below. Be sure to specifiy the product and size your clinician has recommended you to try.

2) One (1) box of product will be shipped upon enrollment.

3) A Coloplast Care advisor will call you within five business days, to welcome you to the program, and confirm your information.

4) Once your information has been confirmed, the Care Advisor will assist you with your reimbursement and funding needs.
